📌 Participant Waiver – Required Agreement

📌 By submitting this form, you agree to the following terms: 1. Assumption of Risk I understand that participating in martial arts activities, including training with UFC athletes at East LA Jiu-Jitsu, involves inherent risks, including but not limited to personal injury, muscle strains, fractures, or unforeseen incidents. I voluntarily assume all risks associated with my participation in this event. 2. Health & Fitness Confirmation I confirm that I am in good health and have no medical conditions that would prevent me from safely participating in Jiu-Jitsu training. If I have any existing injuries or conditions, I will notify the event organizers before training. 3. Release of Liability I release and hold harmless East LA Jiu-Jitsu, Boyle Heights Jiu-Jitsu, UFC athletes, event sponsors, coaches, instructors, staff, agents, volunteers, and representatives, from any and all liability, claims, demands, or causes of action arising from my participation, whether caused by negligence or otherwise. 4. Media Consent I grant permission to East LA Jiu-Jitsu to capture photographs and video recordings during the event that may include my likeness. I understand that these images may be used for promotional, educational, or social media purposes without compensation. 5. Priority & Capacity Disclaimer I understand that priority access is given to East LA Jiu-Jitsu students. If the venue reaches maximum capacity, participation may be denied to accommodate our students first.
